Important information

• * For Voice incidents it is mandatory to report the CLI (Ex. 912706000) in the Service Number field.
• * For data incidents it is mandatory to report UserID (eg. PPP001L0) in the Service Number field or SiteID.
• * For wholesale incidents it is mandatory to report Service ID (Service number, CLI, externalID, userID or Administrative) or SiteID, not both.
• * For the rest of the cases it is mandatory to inform the SiteID.
• - If you want to attach a file use the attachment function of your mailing application.
